Children’s Rehabilitative Services (CRS)


What is CRS?

Arizona’s Children’s Rehabilitative Services (CRS) is a program that provides medical treatment, rehabilitation, and related support services to AHCCCS members who have completed the CRS application and meet the eligibility criteria to be enrolled in CRS.

CRS members receive the same AHCCCS covered services as non-CRS AHCCCS members; however, services to treat CRS conditions for Acute Care members may only be provided to children enrolled with CRS. CRS members will be able to receive care in the community or in multispecialty interdisciplinary clinics that bring all specialties together in one location.

Services are provided for the CRS condition as well as all other medical and behavioral health services for most CRS members. This integration of CRS, medical and behavioral health services for most CRS members is a change that will become effective October 1, 2013.

Who Can Apply for CRS?

AHCCCS members under the age of twenty-one (21) are eligible for enrollment in the CRS Program when there is presence of a CRS-covered condition requiring active treatment.

You cannot be enrolled in the CRS program until you become an AHCCCS member. Anyone can fill out an application including a family member, doctor, or health plan representative.

To apply for the CRS program, a CRS application, either in English or Spanish, needs to be printed, filled out and mailed or faxed with medical documentation that supports the potential CRS condition to the CRS Enrollment Unit.

If Approved For CRS, Who Will Manage Care for CRS Conditions?

UnitedHealthcare Community Plan CRS is the AHCCCS health plan that will manage care for CRS conditions, acute health and behavioral health services. There may be exceptions for some CRS enrolled members. For specific coverage types, click the link below.

ATTENTION PROVIDERS:

Information regarding member’s eligibility for CRS will not be complete in the member verification application until 10/10/2013. In the meantime, CRS data can be obtained from the 270/271 or by calling the AHCCCS Communications Center.
