Excluded Providers

Pursuant to the regulation 42 CFR 1002.212, if a State agency initiates an exclusion pursuant to the regulation 42 CFR 1002.210, the State must provide notice to the individual or entity subject to the exclusion, as well as other State agencies; the State medical licensing board, as applicable; the public; beneficiaries; and others as provided in 42 CFR 1001.2005 and 42 CFR 1001.2006.

Office of Inspector General is in the process of creating a searchable database. The following list contains Medicaid providers who have been excluded by AHCCCS from participating in Arizona’s Medicaid agency.
