Fee-For-Service Formulary for Providers

The AHCCCS approved drug list, called the Fee-For-Service (FFS) Formulary, is used by members and providers and lists all covered drugs for the FFS program, along with any indicators of special requirements. The formulary provided below is not applicable to health plan members. Health plan formularies can be found in Health Plans.
