This website provides a variety of reports on a regular basis, including:

If you are interested in information that is not available on this website, you must use the Request for Data/Information Form to submit a custom request. The completed form must be accompanied by a signed Conditions of Use Form.

AHCCCS responds to custom data requests on a cost-recovery basis. Pricing is set at $175/hour, plus mainframe processing fees when appropriate.
