If you or a loved one is enrolled with the Arizona Long Term Care System for Elderly and Physically Disabled (ALTCS-EPD) that serves individuals who are aging or have physical disabilities, or if you are an AHCCCS-registered provider serving this population, we want to hear about your experiences.

AHCCCS is working on new health plan contracts for the ALTCS-EPD program and needs your input on what is working well and where there are opportunities for improvement.
